Registration

We will be accepting 2008 registrations beginning March 1 

 

 

Three Ways To Register

1. By Mail

For CYO:  Secure a camp registration form either from a brochure (call 216-334-2963 to request one) or download the brochure below. Complete the registration form within the brochure and mail it to the CYO Camp Office, 7911 Detroit Ave., Cleveland, OH, 44102

For Happiness:  Call 216-334-2963 or 216-334-2964 and ask for a registration packet to be mailed to you.  Camp Happiness will not accept registrations from the above form.

3.  In Person at our Office:

You may come to our office at 7911 Detroit Ave. (Disability Services office is on the ground floor) Mondays-Fridays, 10:00 am to 4:00 pm to request registration material.

Please call first to be sure someone is available.

Additional CYO Camper Forms

In addition to the above registration, each camper must complete some additional forms.  You can do this after you are confirmed for camp.  For the protection of your child, all camper forms must be completed and returned at least two weeks prior to camper’s first week.  Unpaid camp fees and incomplete or missing forms will prevent your child from participation in the program.

Payment Procedures

Your child’s weekly registration must be pre-paid.

           Week 1     payment due before     June 2

           Week 2     payment due before     June 9

           Week 3     payment due before     June 16

           Week 4     payment due before     June 23

           Week 5     payment due before     June 30

           Week 6     payment due before     July 7

           Week 7     payment due before     July 14

           Week 8     payment due before     July 21

The Terms--Cancellations and Refunds

The Catholic Charities Camping Programs operate on a first come first serve basis.  When necessary we do maintain a waiting list.  However, we reserve the right to cancel any program at any site due to lack of enrollment or interest.   If Catholic Charities cancels the program, either an alternate site will be offered or all fees paid will be refunded. 

 

You are financially responsible for all weeks registered. If you should need to cancel a week, at least two weeks notification is required.  Failure to notify the office by phone, email or fax at least two weeks prior to the scheduled camp session will result in your financial responsibility for that week.  You can call the office at 216-334-2963; email  mjscott@clevelandcatholiccharities.org or fax to 216-334-2905.

 

Notifying a camp counselor or site director of your intention to add/drop/change weeks is not sufficient, YOU MUST MAKE ALL CANCELLATIONS OR CHANGES through the office  or you will be held financially responsible for that week.

 

REFUNDS: There will be no refunds given except for uncontrollable circumstances after the two week deadline.  All requests for refunds must be in writing, and include a description of the reason and circumstances for the request.  A doctor's note is required for multiple day absences.  Not all requests will  be granted. If a refund is granted, Catholic Charities will refund all fees less a $50 fee.  No refunds will be given for absence for personal reasons.  Weeks will not be prorated for days missed.
