Camper Forms

CYO Campers...We will need the the following forms for each individual camper:

• Health History (PDF)- Completed, signed and dated by parent/guardian

• Personal Profile Form (PDF)- For the staff to read to better know your child

• Statistical Form (Word)

• Payment Coupon Voucher Form (PDF)

• Pre-camp survey- to allow us to gather outcome measurements (still to come)

Not a form but required reading:

• Parent Handbook (PDF)

Your cooperation in completing and returning these forms will be greatly appreciated, as no child may be in attendance without all forms on site.
