• Between January 1 and March 15, 2011, submit Form A to YYAM Office with $90 per person deposit is required. Space is limited and will be filled on a first-come, first-served basis Deposits are non refundable.
• Once Form A is received by the YYAM Office, the diocesan leader will receive an excel spreadsheet via email to track their individual registrations. Hardcopies are available upon request for those without computer access.
• Apply for Youth Animator Program as desired, deadline for application is April 15, 2011. Deadline to compete for a Diocesan Animator Scholarship is March 15, 2011. Only four scholarships are available.
• Apply for NCYC scholarships. See deadlines. Scholarship monies are limited.
• Gather signed NCYC Liability Waivers and Diocesan Medical Release forms for all youth participants and adult chaperones. Submit to YYAM Office by June 29, 2011.
• Complete parish housing request (Form B) and return to YYAM Office by June 29, 2011. $200 pp payment is due.
• Submit adult chaperones Virtus material to YYAM Office by June 29, 2011 for verification.
• Keep track of payments using the Final Payment Worksheet. Submit Form J with final payment to YYAM Office by September 1, 2011.

You must make and keep copies of all forms submitted to the Diocesan Office, including all waivers and medical forms. Please consult with your parish leadership to determine if parish-specific permission forms are required. The YYAM Office does not need copies of any parish forms you may develop.

Youth Animators
The Youth Animators Formation Program is designed to form young people to take lead roles in general session programming at the 2011 NCYC, especially as prayer leaders.   This is an optional program with additional costs.  Four diocesan scholarships will be awarded.

Virtus Proof of Compliance			Due 6/29
All Adults attending must submit proof of compliance.  To obtain Virtus forms, have every adult and young adult over 18, log in to the Virtus website  http://www.virtus.org/virtus/ and print a copy of their completed training record.  This printout must be sent with all registrations by June 29th.  If your adult volunteers, or young adults over 18, have not yet completed Virtus training please have them sign up and complete this training ASAP.  The Diocese of Cleveland and NFCYM will not allow adults to attend NCYC until they have complied.
