2007 NCYC REGISTRATION INFO

You must make copies of each of these completed forms before sending them to the Youth & Young Adult Ministry and CYO Office. 

  • All youth must be in 9-12 grade during the 2007-08 school year to attend as a youth participant.

  • Chaperone to youth ratio is 1:5. Chaperones must be over 21, provide proof of Virtus Training. and be screened using the forms provided. Every parish group must have a minimum of 2 chaperones.

  • Those between 18-20 who have graduated from high school and act in a leadership capacity within a parish youth ministry program on a regular basis are invited to register.  All must comply with parish chaperone leaders and diocesan staff.

  • Protecting God’s Children:  All adults, 18 and up, must show compliance with the Protecting God’s Children policy.  A copy of the Virtus attendance certificate and the date of completion should be sent to Mary Ellen Ewers-Peña, x24 by e-mail:  meepena@clevelandcatholiccharities.org or the address below:

Registration materials are available for download below. 

Form A   182 KB   NCYC Parish Registration Due between 1/1 - 3/31
Complete and send to the Youth & Young Adult Ministry and CYO Office with a $75 per person deposit to save your seats.  
Form B  138 KB  Parish Rooming List Due 6/29 
Youth rooms will have up to 4 in each and adult rooms 2. No single rooms are available due to space limitations. You may place either 1, 2 or 4 youth in a room (not 3) so that other singles or doubles can be paired together.   
Form C  32.1 KB  Change/Delete Form Due with each change up to 10/20
Used with any and all additions, deletions, changes and substitutions made after June 29 (once Form F has been returned to the Youth & Young Adult Ministry and CYO Office). Only substitutions accepted after September 14.  
Form D 126 KB  Diocesan Medical Release Due 6/29
Must be completed by ALL participants (youth, young adults and adults) with three copies made; one sent to the Youth & Young Adult Ministry and CYO Office, one kept by the parish or small group leader and one kept by the pilgrim in their name badge during the conference.  
Form F      Individual Registration (Excel Spreadsheet) Due 6/29 
List all people attending from your parish on this form (group leader, adults, young adults and youth). The Excel spreadsheet will be sent to registered parishes via e-mail  with instructions on how to complete it.  

A Word file format of Form F is available for registered parishes preferring to mail/FAX a hard copy for everyone attending from your parish.

Only those requests made by June 29 will be considered. Parishes and applicants will be informed of actual awards by July 30.   Additional money will be due by September 14.

Use this form to tabulate the final payment and send with all remaining forms/changes. No new registrations will be accepted after September 14. Each change after June 29 will cost $10.

$430 per person Conference Registration Fee Includes:
  • Full access to all NCYC Conference programs and event sites.
  • 3 nights at the Hampton Inn in Dublin, OH, our diocesan hotel featuring hot breakfast; pool; in room coffee maker, iron and ironing board; some meeting space and is wheelchair accessible.   ROOMING NOTE: Youth will be placed up to 4 to a room with 2 double beds.  Adults will be placed 2 per room with other adults or young adults.
  • Transportation, some meals, tour, t-shirts, trading items and the experience of a lifetime!
  • Regular informational updates via e-mail plus ongoing support and guidance and assistance from our expert NCYC Pilgrimage Team

REGISTRATION/PAYMENT DEADLINES

v      Parish Registration Form A and Initial Deposit     $75 per person          Jan. 1-March 31

v      Second Payment with Permissions/Rooming List   $200 person              June 29

v      Final Payment - All forms must be turned in          Remainder of Sum     September 14

 

 DIOCESAN FEES REGARDING CANCELLATIONS, REFUNDS AND CHANGES

  • Each change (substitution, addition or deletion) between June 29 and September 14 will be charged a $10 processing fee.

 

AFTER SEPTEMBER 14, THE FOLLOWING DIOCESAN FEES GO INTO EFFECT

  • Additions/Substitutions between September 14 - October 20 will be charged a $10 fee
  • Cancellations (without substitutions) between September 14 - October 20 will be refunded minus a $75 cancellation fee.
  • Additions/Substitutions after Oct. 20 will be charged a $25 fee
  • Cancellations after Oct. 20 will be refunded minus a $150 cancellation fee.
  • Any name changes made after October 20 must be done upon arrival in Columbus directly with NFCYM at the NCYC Registration Area in accordance with any NFCYM fees & policies in addition to the $25 Diocesan fee listed above
