Purpose: A supervised day program that provides necessary care, activities, respite, and support for older adults and their caregivers. An interactive program that addresses the physical, mental, emotional, social, and spiritual needs of participants. Activities designed to maintain participants at an optimal level of functioning, to enhance their quality of life and ability to remain in the community.
Population Served: Older adults needing help with basic living skills or requires supervision due to cognitive decline, diminished abilities, or functional limitations. The program also addresses the needs of those at risk of social isolation due to multiple losses associated with advancing age. The program serves both the participant and caregiver by providing care and support to help manage the tasks of daily life and the associated stress of meeting these needs.
Criteria: Adults and Caregivers must participate in the initial intake/assessment process with the program Social Worker and RN. All participants must have a current TB test (within the past 12 months prior to enrollment). Participants must be able to stand or pivot with assistance (the program cannot accommodate those that require a total lift). Clients may attend on a fulltime or part-time basis.
Fees: The private pay rate is $42/day. Transportation provided for additional $6.00. Financial assistance may be available for those who qualify.
