St. Augustine Health Ministries

Founded in 1969 by Catholic Charities of the Catholic Diocese of Cleveland, St. Augustine Manor’s purpose was to provide quality long-term care to the area’s aging population. Since its origins, it has evolved into St. Augustine Health Ministries, a multifaceted organization serving people of all ages, backgrounds and walks of life through skilled nursing, rehabilitation, assisted living, long term care, hospice care and a child enrichment center.

The continuum of care provided by St. Augustine Health Ministries celebrates and empowers each person to live a life of purpose, independence and well-being. Embraced by a friendly atmosphere upon entering, each resident is immersed in a variety of pursuits, with an emphasis on a “life-giving”, not “life limiting” environment. The goal is to enhance the social, emotional and spiritual well-being of each person, regardless of religion or financial status.

Programs and Services



St. Augustine Manor

Purpose: To provide skilled nursing, inpatient and outpatient rehabilitation, sub-acute care and long-term care in a resident-centered, faith-based environment. Specialty areas include stroke, traumatic brain injury and Multiple Sclerosis (MS). Dialysis is provided on campus.

Population served: Individuals ages 18 – 100+ years with a variety of illnesses and physical challenges.

Criteria: None.

Fees: Private pay, most insurance, Medicare and Medicaid.

Major funders: Medicaid; Catholic Charities Annual Appeal

St. Augustine Towers

Purpose: To provide gracious and affordable assisted living with a variety of apartment sizes and styles. Inclusive rents, no enrollment or hidden fees.

Population served: Active seniors, 55+

Criteria: Private pay, assisted living waiver program, VA benefit.

Fees: Monthly rents between $800 - $2,250

St. Augustine Manor Child Enrichment Center

Purpose: To provide quality programming in a safe environment that nurtures the child and supports the family.

Population served: Infants 6 weeks old through pre-school.

Criteria: Cuyahoga County vouchers accepted.

Fees: $125 – $197 per week.

Holy Family Home

Holy Family Home Website

Purpose: Established in 1956 to provide-end-of-life care with compassion and grace in a comforting and peaceful environment conducive to the patients’ needs while maintaining their dignity.

Population served: Residents of Northeast Ohio.

Criteria: Must have physician certification of terminal illness and life expectancy of six months or less given a normal course of the illness.

Fees: None.

Major Funders: Funded significantly by donations from families of the deceased and the community.

Holy Family Hospice

Holy Family Home Website

Purpose: To provide hospice services to individuals who reside at home, in a nursing home, or in an assisted living community. Holy Family Hospice offers a holistic approach to patient care with a focus on spiritual care, regardless of a person’s faith preference.

Population served: Residents of Northeast Ohio.

Criteria: Must have certification from a physician stating that patient is terminally ill and, given the normal course of the treatment process, the patient has a life expectancy of six months or less.

Fees: Medicare, Medicaid, private pay and private insurance.

Major Funders: Donations from families of the deceased and the community.
