St. Martin picture St. Martin de Porres Family Center originally started providing services in 1967 from a storefront on East 105th Street in Cleveland and in 1976 moved to its current location in Cleveland’s Glenville/Forest Hills neighborhood where services are provided to all ages.

Programs and Services



Moms First

Purpose: To provide health education and service coordination to expecting mothers during their first trimester through age 2 in order to reduce the Northeast Ohio infant death rate.

Population Served: Mothers in their first trimester of pregnancy.

Criteria: Expectant mothers residing in Cleveland.

Fees:There is no charge for this program.

Major Funders: City of Cleveland Department of Public Health.

Help Me Grow

Purpose: Provides a continuum of care for children as they prepare for school enrollment, to ensure they receive immunizations; and to teach parents the skills to identify whether their child has healthy development.

Population Served: Parents with infant children.

Criteria: Children aged 0-3.

Fees:There is no charge for this program.

Major Funders: Cuyahoga County of MRDD.

After School Program

Purpose: To provide after school programming to enhance academic performance, improve social skills, recreational activities and snack. Youth can also express themselves through Martial Arts instruction.

Population Served: Youth ages 6-21

Criteria: Complete registration form.

Fees:There is no charge for this program.

Major Funders: City of Cleveland Community Development Block Grant

Tapestry

Purpose: To work with children and families who have serious emotional needs which severely disrupt relationships and daily activities.

Population Served: Children diagnosed with severe emotional disorders (SED)

Criteria: Must undergo a diagnostic assessment and be assessed as SED

Fees:There is no charge for this program.

Major Funders: Mental Health Board

Senior Program

Purpose: To provide a safe program to older adults through adult day care, adult development, transportation, congregate meals, home delivered meal, and supportive services.

Population Served: Seniors over the age of 60, residing in the Glenville, Collinwood and Forest Hills area.

Criteria: Age 60+ and must meet income guidelines.

Fees:Self pay if income exceeds income guidelines.

Major Funders: Department of Senior and Adult Services; Western Reserve Area Agency on Aging, City of Cleveland Community Development Block Grant.

Emergency Assistance

Purpose: Designed to help children and families in need, as well as individuals who are experiencing some hardships. Provides assistance with food, rent and utilities.

Population Served: Low income residents living in the 44108 zip code.

Criteria: Must provide proof of identity and size of household. For rental assistance, must provide name of landlord, SSN and proof of ownership.

Fees:There is no charge for this service.

Major Funders: Catholic Charities Annual Appeal; TANF; FEMA and private donations.

Community Wrap-Around

Purpose: Designed to meet the needs of children and families involved in multiple systems that are not being successful in addressing their needs. The process is driven by the voice and choice of the families who agree to participate, identifying the strengths of the family and developing a plan to address their need by coordinating supports, both formally and informally.

Population Served: Families residing in the Glenville, Collingwood, Forest Hills area who are involved with multiple systems.

Criteria: Families involved with multiple systems.

Fees:There is no charge for this service.

Major Funders: Cuyahoga County Children and Family Services.
